There are two different medical forms to choose from - please read the information below and choose the appropriate form for your individual circumstances.
Adult Pre-Exercise Screening Tool & Doctor's Approval Form Information
All existing and ongoing pool users can complete the adult pre-exercise screening tool which is provided below as a pdf file. This is a self assessment medical form which is to be handed in to your pool co-ordinator at the beginning of each year. This will be the default medical form if you have ticked "NO" in all sections. It is a two sided form so don't forget to fill in the second page before you hand it in.
If you have ticked "YES" to any questions on the pre-screening tool medical form you will need to book a medical review and have your doctor complete the Doctor's Approval Form which is also provided below as a pdf file. This needs to be handed in to the pool co-ordinator of your relevant pool at the beginning of the year or when commencing classes again.
If you are a new member or prospective member of the Warm Water Exercise Program you will need to have a medical review and a completed Doctor's Approval Form handed in to the pool co-ordinator before commencing any classes or trials.
People’s circumstances and health status can change from one year to the next and it is important to have up to date medical information for the safety of the program participant and also leaders of the programs, so any medical issues which may arise can be dealt with appropriately.

We received a grant from Fosterville
Gold Mine, October 2019 to
purchase a defibrillator.
- We received another grant from Fosterville Gold Mine, CharitABLE Grants, in September 2020 to help clubs financially through lack of fund raising during COVID 19. We are very grateful again for their support.
- We are very grateful to the City of Greater Bendigo for a grant which has assisted us to subsidise our pool fees. This has helped with getting people back to warm water exercise programs following COVID restrictions.
